Reports to: Senior Social worker

Responsible for: Supervision of an assistant social worker and/or social work student.

Job Description

Job Purpose

To support services the curative/hospital based, primary, preventive and promotional programmes and activities; counseling of patients, families and the community at large; and providing psychosocial rehabilitation, home based care and placement of patients to improve the quality of life by empowerment and education.

Key Outputs:

• Spent a minimum of 80 % of time on patient related activities
• Patient treatment goals achieved.
• Patient medical records including case notes are maintained in the appropriate format and in a timely manner
• Achievement of monthly Social Work targets
• Evaluation of service provision and quality improvement activities are undertaken and results documented.
• Organizational policies and procedures are understood, followed and reflect current practice, including risk assessment and notification
• Attendance at and contribution in area of expertise, at multidisciplinary meeting if and when required

Key Functions: This is the operational level, the level of independent professional practice. Under supervision, officers perform the full range of social work duties.

• Undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate intervention and discharge plans
• Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes
• Actively participate in quality improvement and research activities
• Provide supervision and participate in clinical education programs as required
• Promote and develop positive relationships within the department, wider organization and with community partners

Person Specifications

Qualification

1. Master’s Degree in Social Work or relevant field (Sociology, Psychology, Anthropology, Mental Health or its equivalent qualification from a recognized University).
2. Degree in Social Work or relevant field (Sociology, Psychology, Anthropology, Mental Health or its equivalent qualification from a recognized University).
3. HND in Social Work or relevant field (Sociology, Psychology, Anthropology, Mental Health or its equivalent qualification from a recognized University).
4. Diploma in Social Work or relevant field (as above)

Experience

With HND and Diploma, experience is as follows:

• plus two years satisfactory work experience as Assistant Social Worker.
• plus 3 years experience as Assistant Social Worker

Competencies

• Technical
  • Demonstrated clinical skills knowledge and reasoning in psycho- social assessment and goal based interventions in an acute setting
  • Demonstrated ability to work both independently and as part of a multidisciplinary team
  • Demonstrated the understanding of the principles of discharge planning
  • Experience in accessing a variety of community resources and systems
  • An understanding of the principles of continuous quality improvement and demonstrated skills and initiative in service development, evaluation , quality activities and / or research
  • Established skills in organization, planning and priority settings
  • Demonstrated computer skills
  • Demonstrated commitment to the maintenance of case notes in medical files and hand over notes and to the timely input of all data requirements
• Behavioral
  • Excellent interpersonal skills and communication skills including proven ability to interact with service users and their families in a caring and sensitive manner.
  • Positive working attitude and ability to take instructions.
