Post Title: Social Worker

Section: Treatment, Rehabilitation & Reintegration / Harm Reduction (depending on deployment)

Reports To: Programme Manager / Unit Head

Responsible For: N/A (may supervise interns / outreach workers where applicable)

Job Description

Job Purpose:

To provide professional social work services including assessment, case management, counselling support, and reintegration assistance to individuals affected by substance use, ensuring holistic care and support in line with APDAR’s mandate.

To support prevention, treatment, rehabilitation, and harm reduction programmes through client-centred interventions, coordination of services, and community engagement.

Key Outputs:

  • Client assessments and case management plans developed and implemented
  • Clients supported through rehabilitation and reintegration processes
  • Referrals and coordination with partner agencies conducted effectively
  • Client records and reports maintained accurately and timely
  • Community and family interventions conducted where required
  • Compliance with case management protocols and ethical standards ensure
  • Contribution to programme outcomes (treatment, reintegration, harm reduction) achieved

Key Functions

  1. Conducting psychosocial assessments of clients and identifying needs and risks
  2. Developing and implementing individual case management and care plans
  3. Providing counselling support and psychosocial interventions within scope of practice
  4. Supporting client rehabilitation, recovery, and reintegration into society
  5. Coordinating referrals to health, social, legal, and community support services
  6. Liaising with families (through home visits), communities, and partner institutions to support client outcomes
  7. Participating in multidisciplinary team meetings and case reviews
  8. Maintaining accurate and confidential client records and documentation
  9. Preparing case reports, progress reports, and follow-up documentation
  10. Supporting outreach and harm reduction activities where required
  11.  Conducting awareness, education, and sensitisation sessions
  12. Monitoring client progress and adjusting intervention plans accordingly
  13. Ensuring compliance with professional ethics, confidentiality, and safeguarding standards
  14. Identifying risks and escalating safeguarding or protection concerns
  15. Participating in programme monitoring, evaluation, and reporting activities

Job Requirements

Person Specification

  • Qualification
    • Degree in Social Work from a recognised institution
      OR
    • Diploma in Social Work with relevant experience
  • Experience
    • Minimum of one (1) to three (3) years relevant experience in social work or a related field
      OR
    • Successful completion of a recognised work attachment / practicum with APDAR or a similar institution during training
  • Advantage:
    • Experience in substance use, rehabilitation, community work, counselling, or related social services
